“Philtrum reduction” in Korea can refer to several different operations. A subnasal incision, a lip-line incision, a hybrid plan and an intranasal suture-fixation technique change different areas and leave incisions in different locations.
1. What does philtrum reduction actually change?
Assessment should not rely on one nose-to-lip measurement. The surgeon also reviews upper-lip show, philtral columns, Cupid’s bow, tooth show when smiling, the alar base and mouth-corner direction. Published studies use different measurements, so there is no universal “ideal” length.

2. Subnasal, lip-line, hybrid or suture fixation?

| Method | Main goal | Typical incision or area | Do not confuse it with |
|---|---|---|---|
| Subnasal lip lift | Shorten the central distance and increase upper-lip show | Along the nostril sill and alar base | A mouth-corner lift |
| Lip-line lift | Address length and a thin upper lip | Along the vermilion border | A subnasal scar; the scar sits elsewhere |
| Hybrid reduction | Treat central length and lateral upper-lip roll | Central subnasal plus lateral lip design | A fixed package; it must be tailored |
| Intranasal suture fixation | A subtler change in length or lip eversion | Small intranasal incisions with deeper fixation | The same degree of change as skin excision |
| Lateral lip or corner lift | Adjust the lateral lip or mouth-corner direction | Around the lateral upper lip or mouth corner | Central philtrum shortening |
Do not ask only how many millimetres will be removed. Ask how the central and lateral lip will change, how much red lip will show and whether mouth-corner lifting is a separate part of the plan.
3. How much does philtrum reduction cost in Korea?
This is a reference for a standalone philtrum-reduction procedure. Hybrid surgery, mouth-corner lifting, revision surgery or a combined rhinoplasty requires a separate quote.
Confirm the incision, anaesthesia, 10% VAT, suture removal, medication and scar-care charges in writing.
4. Scars and recovery
Any procedure that removes skin leaves an incision. A subnasal incision is positioned along the nasal base to make it less visible from the front, but tension, skin type, aftercare and individual scar tendency still matter. Alar-base shape, nostril show, asymmetry and under-correction also need discussion.
5. Eight questions to ask at consultation
- Is my concern true length, an inward-rolled lip, mouth-corner direction or a flat philtral groove?
- Why is subnasal, lip-line, hybrid or suture fixation recommended?
- How much will the central and lateral areas change?
- Where exactly will the incision run, and how may the alar base or lip border change?
- Is my current upper-tooth show suitable for shortening?
- How do previous rhinoplasty, alar reduction or lip filler affect the plan?
- When can I have sutures removed, wear makeup, speak and eat normally?
- Who follows scar thickening or asymmetry after I return home?
6. Who needs a more careful assessment?
People with substantial upper-tooth show, a naturally short upper lip, alar-base asymmetry, a history of raised scars, recent rhinoplasty or revision surgery need a more detailed review. Removing more does not automatically create a better result.
Frequently asked questions
How long does a philtrum need to be before surgery?
There is no single cut-off. Lip shape, tooth show, the alar base and overall facial balance matter together.
Is a subnasal lip lift the same as a mouth-corner lift?
No. One changes vertical distance; the other changes corner direction. They may be combined but remain different operations.
Can groove contouring replace shortening?
No. It improves definition but does not directly remove skin to shorten the distance.
Can it be completely scarless?
Skin-excision surgery always has an incision. Placement can reduce visibility, but no-scar claims are unrealistic.
Can I have it after rhinoplasty?
Sometimes, but previous incisions, the columella, alar shape and implant or cartilage position must be assessed.
